TEST-ConSaf Permit Request

Complete this permit request well before the start of a contractor project/late into planning. This will be sent to the BCIT HSE team for review and approval. Once approved, you can attach the approval notification/email to any future submissions to SSEM for access, shutdowns, or other requests to indicate that the project scope has undergone HSE review and approval.

Some additional suggestions and notes:

1. Be liberal with requested date range.
2. Attach plans or other documents that help demonstrate answers to relevant permit questions (e.g. impacts).
3. For locations - you can select from campus, building, down to individual room numbers. You cannot select multiple - so use the "additional location details" fields to provide further details/information.
4. Overall goal is to demonstrate that the project has identified and planned for hazards and impacts to the institute, and that these will be adequately managed. Contractor compliance to OHS regulations is not BCIT's direct responsibility.
